Separate routine questions from decisions that can wait no longer
A useful exception report tells the business which unresolved issues could affect reporting, payments, compliance, or an immediate decision. It should not repeat every request in the bookkeeping inbox. Keep routine document collection in its own queue and escalate the items that need judgment, approval, or a time-sensitive response.
Describe each issue with the account or workflow, amount where known, affected period, missing fact, and consequence of delay. Replace vague labels such as needs attention with a question the responsible person can answer.
Consider the nature of the issue as well as its size
Agree review priorities with the accounting owner and relevant specialists. A small unusual payment, an owner transaction, or a missed filing task can matter for reasons beyond its dollar amount. A large fully explained timing difference may need monitoring rather than urgent correction.
Do not use a single percentage as a universal materiality rule for every business or reporting purpose. The appropriate assessment depends on the facts and framework. The exception report can apply operational priorities without pretending to replace a professional materiality judgment.
Give the reader a proposed next step and an owner
For a missing supplier invoice, name the purchaser who can obtain it. For an accounting policy question, assign the reviewer. For a delayed payment, identify the person authorized to contact the customer or provider. A shared list without ownership often becomes a record of problems rather than a way to resolve them.
An illustrative entry might state that a $6,000 processor balance has not settled, the provider's review date is unconfirmed, and the cash forecast currently assumes no release. The next action is to obtain verified status and reassess the forecast, not to categorize the balance as an expense merely to clear the report.
Show what changed since the last review
Track newly raised, resolved, overdue, and repeated issues. Retain the evidence and decision when an item closes. A resolved label should explain whether the transaction was corrected, the missing document arrived, or an approved provisional treatment was adopted.
Use recurring issues to improve the source process: expense submission, contract handoff, integration mapping, or provider response. Keep the report brief enough for decision-makers to act on, with detailed evidence linked in the controlled working records. Its value comes from the decisions it prompts and the uncertainty it makes visible.
