Establish what changed and why
Identify the new evidence, original entry, affected entity and period, and proposed correction. Determine whether the issue is an error, estimate update, timing difference, or accounting-policy question. Have the qualified reviewer assess the applicable treatment.
Do not assume every late invoice belongs in the current month or that every discrepancy requires reopening history. The reporting framework, facts, and materiality assessment matter.
Preserve the released version
Keep the original statements and reporting pack with their release date. Record the correction separately and identify the revised version. That trail explains why a figure in an old board pack differs from the current ledger.
For illustration, a material supplier bill received after a month closes may affect that month's expense under the relevant policy. Posting it silently without updating the reporting record leaves management comparing different versions.
Approve the accounting change
Document the amount, accounts, calculation, period, supporting evidence, preparer, and reviewer. Confirm who may edit a locked period and how the application retains its audit trail.
If an integration or subledger created the discrepancy, correct the source process where appropriate. A ledger-only repair can be overwritten or duplicated by the next sync.
Check downstream reports and filings
Review whether management reports, lender information, investor packs, tax handoffs, or filed returns need attention. Ask the responsible specialist to assess legal or tax reporting consequences.
Maintain a distribution record for revised reports when relevant. A correct accounting file does not prove that everyone relying on the earlier figures received the correction.
Prevent the same late adjustment
Identify the cause: missing source records, unclear cutoff, unreviewed estimates, or delayed approvals. Update the close checklist and input ownership.
Retain an adjustment log that distinguishes routine estimates from material corrections. The next reviewer should be able to understand both the final figures and why the previously issued version changed, without relying on an oral explanation.
